FSA Policy for Releases of Producer Information

The following policy has been established by the Nebraska State FSA Office as guidance for the county FSA staff to follow so that a uniform method of information delivery is being followed statewide.

FSA-426 requests are only allowable forms of obtaining information for Crop Insurance Adjustors.  Crop Insurance Agents submitting FSA-426 forms requesting information will have the request denied.

Producer information cannot be released to an insurance agent without the express written consent of the producer.  The written consent can be in the form of a Certification of Identity.

  • Certification of Identity forms are “one time use” forms, they cannot be left on file for future requests.
  • FSA offices have 30 days to provide the requested information.

Once written consent is obtained, only the data applicable to that specific producer will be released and all PII (personally identifiable information) will be redacted by the FSA office prior to providing it to the crop insurance agent.  PII data includes such things as social security numbers, mailing addresses, telephone numbers, email addresses, etc.

The extent of the information to be released must be for specific data types.  For example:  specific farm numbers; the crop year requested; whether or not to release aerial photos; etc.  Requests for “all information” will not be honored.

There are fees per page associated with these releases of information that you will be charged. The fee for aerial photos is $1 each and all fees must be paid in advance of the information being provided.

Producers will be provided applicable requested information, free of charge.
